Purchase Requirements: To receive business to business pricing you must be in a business related field or have a business related enterprise, provide your signed current year W-9 and signed current year Tax Exemption certificate along with the other information requested in the Van Dyke’s Contractor/B2B Buying Application form below. Acceptance into the Contractor Program will be dependent upon our receipt of the appropriate and up-to-date documents.
